Dark tan booklet with black ink. On the front there is an eagle at the top. There is circular type under the eagle that reads, "FOR A STRAIGHT TICKET MARK WITHIN THIS CIRCLE" within the circle is an "X". A list of candidates for National Office is listed below.
On the back cover is a list of the voting district/precinct/ward/township/county. The handwriting in blue ink is for the most part faded and illegible, however we can make out that this book was used in "Erie Township".

This voting report instructs school district, polling district or other special committeemen to: "Carefully poll the election district assigned to you, and enter in this book, with the least possible delay, the names of all the voters in your school, polling district, or the special territory over which you have charge, designating the politics of each, as follows:
Republicans, R; Democrats, D; National Party, N; Prohibitionists, Pro.; Doubtful Republicans, D.R.; Doubtful Democrats, D.D.
Be careful to give P.O. address, occupation, etc., of each voter.
When complete, copy the names and politics of each into a duplicate of this book, and send the copy to your Township, Ward or Precinct Committeeman, as the case may be, keeping this book for your own use. . . "

Ohio Statehouse Museum staff member, Katie Clement, purchased this item at an antiques store in Marblehead, Ohio, in June 2015 and donated it to the Capitol Square Review and Advisory Board in the same month and year.
The Capitol Square Review and Advisory Board took over the care of the Statehouse and its collections in 1988.
